Full Moon Farm's Liability Release Form

hereafter referred to as the RELEASER, executes this release/contract on this date of                .           This Release Agreement/Contract is made in favor of Full Moon Farm, 4326 Louisville Road, Finksburg, MD 21048/Stephen/Karen Fulton, including all their employees, successors, assigns and legal representatives.

            The consideration for this Release Agreement/Contract is the permission given to the Releaser to use or be on the premises of Full Moon Farm (at above address) for the purposes of boarding horses or ponies, riding horses or ponies, taking riding lessons, learning horsemanship, showing or schooling horses or ponies, participating in or observing a show or and event, birthday party, field trip, camp, clinic and/or taking part in or observing any activity on the premises of Full Moon Farm.

The Releaser, in signing this Release/Agreement/Contract is fully aware that horse related activities may expose the Releaser to above normal risks and that horses and horse related activities are unpredictable and potentially dangerous.

            This Release Agreement/Contract binds not only the Releaser with regard to any claims arising from participation in these activities but also the Releasers spouse, heirs, legal guardian, legal representatives and assigns.

             In considerations of the above stated purposes and the Releaser's permission to be on the premises of Full Moon Farm, the Releaser forever releases, discharges and covenants to hold the Releasee, their employees, successors, assigns and legal representatives harmless from any and all claims, demands, costs, expenses, loss of services, acts and causes of action which may arise from any act or occurrence on account of any personal injury, disability, property damage, loss or damages of any kind sustained or that may hereafter be sustained by the Releaser in consequence of any act that may be occasioned on account of Releaser being on the premises of the Farm or that Releaser may sustain because of Releaser's proximity to horses and horse-related activities.

I, the Releaser, have read the above Release Agreement/Contract prior to signing this document and I understand the contents and agree to abide by the conditions outlined in this Release Agreement/Contract.

NOTICE: The helmets provided by FMF are made available for your convenience only. Full Moon Farm makes no representations or warranties regarding the extent these helmets will protect you against injury as a result of any impact, accident or fall. FMF strongly recommends that you obtain your own properly fitted protective riding headgear meeting current safety standards and that you wear it whenever you ride. Approved headgear is mandatory at any time mounted on a horse at Full Moon Farm.
